Had this last week, @OP — our audit bot thought a last-name dash was a checkbox cosplaying incognito. I added a tiny pre-check to ignore hyphens in name fields and do a 10‑second glance at any ‘single unchecked box’ flag before reindexing; just watch it doesn’t mute a real missed initial.
@OP I’ve tamed that “missing initial” ghost by setting our audit tool to ignore any checkbox candidate smaller than 12x12 px, which keeps hyphens on name lines from getting flagged and saves me from re-indexing 112-page stacks for nothing. Caveat: some older VA forms use tiny boxes, so I drop the threshold to 10 px on those templates.
